| |
- Applicant's full name, email address, phone number, and address |

We will reply via email with a RMA number. |

Once the RMA number is received, please follow the packing instructions below.

| a. |
Bag the board with an anti-static bag or air cushion foil for better protection.
 |
| b. |
Carefully pack the board into a fitting solid box and make sure there is no room for the board to move around inside the box.
* Please do not return the manual, driver, and accessories. |
| c. |
Please enclose a packing list inside the box. |
| d. |
After packing the card into the box, please label the RMA number on the outside of the package. |
